Our teams are made up of highly skilled and mission-driven healthcare providers who constantly strive to provide the best possible care for our patients and their families.Hourly Range$35/hr - $37/hr SalaryRange $65,520 - $69,264 Agency Placement Fee 25%RegisteredNurse - Home Care- Case ManagerPrimary Staff Nurse is responsible for providing skilled nursingcare to hospice patients. to Hospice, Home Care, Inpatient Care servicesand any other training that may be necessary to complete the assigned duties. Services Team LeaderJob and commit to the Hospice philosophy of care,including the importance of treating the patient as awhole being, physically, psychosocially, and spiritually.Work collaboratively with the hospice interdisciplinaryteam to develop an individualized plan of care for each patient/family, asassigned by the Director of Inpatient Services.Evaluate the patient/family needs, symptoms, and totalcondition in order to report accurately to the physicians and/or Director ofInpatient Services in order that appropriate decisions may be made regardingthe patient/family s care.Contact the patient s primary care physician and/orHospice Medical Director as needed regarding physician s orders for the care plan; he/she is alsoresponsible for continuedcommunication with physicians and other health care personnel concerningchanges in medicaltreatmentand all other aspects of patient care.Initiate nursing procedures appropriate for the patient scare and safety.Teach and counsel the patient/family about the diseaseprocess, medication management, andprovision of care for the patient.Provide hands-on nursing care to the patient andemotional support to the family as needed.Maintain accurate records of Hospice patient/family careservices per Hospice of DavidsonCounty policy.Participate inhospice staff meetings, interdisciplinary team conferences, staff developmentevents, and educational programs.Relate professionally and courteously with patients,families, members of the Hospice ofDavidson County staff and the medical community.Provide on-call nursing services as assigned and committo one 6-hour shift per month as shifts are available.Assist with admissions as assigned.Participate in the Quality Assessment/PerformanceImprovement process and the ACHC accreditationprocess.In emergency situations, the staff nurse may be assigned tothe Hinkle House to assist with patient care.Comply with all HODC policies and procedures, includingattendance and confidentiality standards.Other such responsibilities as assigned by the DCS and/orclinical managers.Abides by the North Carolina Nurse Practice Act.Participate in hospice sponsored in-service training.Physical be able to communicate effectively.Must be able to see, hear, smell, touch asnecessary to assess patients' condition and provide direct care.Continuousstanding, walking, reaching, squatting, bending. Frequent lifting of 50-100 pounds.Status and Hours of position is classified as exempt.
