Job Description:
Registered Nurse/Case Manager Hospice Care in Mishawaka, M-F
8am-5pm, offers a culture of compassion, trust, and professional
growth.
Provides comprehensive patient and family assessments, develops
personalized care plans, and manages pain and comfort.
Collaborates with interdisciplinary teams, supports
decision-making, and supervises LPNs and aides.
Performs about 25 visits weekly, documents care, and educates
patients/families on illness, medications, and hospice rights.
Requires Indiana RN licensure, CPR, valid driver--------s license,
and pass background, drug, TB, and vaccination checks.
