Job Description:

Seeking a Clinical Team Leader for Hospice in New River Valley to serve as a mentor, role model, and resource for staff and operations. Lead and support staff, resolve routine issues, and foster a positive, productive, and fiscally responsible team environment. Implement organizational initiatives, ensure compliance with regulatory and payer guidelines, and maintain clinical competencies. Assess, plan, and evaluate patient care to achieve optimal outcomes across all ages. Coordinate workflow, monitor productivity, and assist with fiscal management. Participate in interdisciplinary team meetings, conduct case conferences, and promote effective communication. Support staff development, handle performance issues, and promote a positive work environment. Requires RN license in Virginia, 5+ years clinical experience, leadership skills, and valid driver--------s license. Minimum 20 hours of annual continuing education; BLS-HCP certification preferred.
