Job Description:
Agape Care Group is seeking a compassionate and dedicated Nurse
Practitioner specializing in Palliative Care to join our team. This
role focuses on delivering patient-centered, specialized palliative
care to patients with serious illnesses, aiming to provide relief
from symptoms, pain, and stress. In line with our mission, you will
play a crucial role in enhancing our patients' quality of life and
supporting their families during critical times. Conduct
comprehensive assessments to determine patient needs and establish
care plans. Collaborate with a multidisciplinary team to support
patient care goals, provide symptom management and improve quality
of life. Communicate effectively with patients and their families,
offering timely information and emotional support. Prescribe
medication and manage therapeutic interventions. Educate patients
and families on disease processes, treatment, palliative care
options, and end-of-life issues. Monitor patient progress and
respond to changes in patient conditions. Participate in ongoing
education, professional development, and research within the field
of palliative care. Strong clinical skills with a focus on
palliative care and symptom management. Excellent communication and
interpersonal skills. Ability to work collaboratively within a
multidisciplinary team. Compassionate understanding of patient
needs and family dynamics. Proficiency in electronic healthcare
record systems. Medical Insurance Dental Insurance Vision Insurance
Life Insurance 401k Current Registered Nurse and Nurse Practitioner
license. Certification in Palliative Care Nursing (CPNC)
preferred.
