Job Description:
Advance Community Healthcare, an FQHC, is expanding our services
and recruiting for a Complex Care Physician who will provide
services to our Advance Careplex at-Beacon Point located in
Raleigh, NC. This role is a unique opportunity provide complex
primary care to individuals, focusing on preventive care and health
maintenance to an ethnically and socially diverse, patient
population in accordance with the values, beliefs and policies of
Advance Community Health. Our interdisciplinary care teams work
closely with patients, leveraging cross-sector partnerships to
deliver patient-centered care tailored to each individual s goals
and priorities. Patients eligible for treatment at Beacon Point
include those with complex medical issues, such as frequent
hospital visits and uncontrolled medical conditions.
