Wound Care Nurse Practitioner / Physician Assistant (Home Health)

Location: Raleigh, Charlotte, NC

Job Type: Full-time

Shifts: Days (Monday-Friday)

Summary:

We are looking for a skilled Nurse Practitioner (NP) or Physician Assistant (PA-C) to provide advanced wound care services to patients across the Raleigh area. This is an excellent opportunity for clinicians who value autonomy and patient-centered care in a home-based setting.

Responsibilities:

• Conduct detailed wound assessments and follow treatment plans
• Track and document healing progress using best-practice protocols
• Provide education to patients and caregivers on wound care techniques
• Coordinate with physicians and interdisciplinary care teams
• Ensure accurate and timely EHR documentation
• Travel to patient homes within Raleigh and surrounding areas

Qualifications:

• Active NP or PA-C license in North Carolina
• Wound care experience is a plus
• New graduates are welcome to apply with relevant clinical exposure or hands-on experience in wound care settings
• Ability to work independently in a field-based role
• Valid driver's license
• Comfortable working in a home care environment
