Overview
Registered Nurse (RN) License required.
This position is responsible for developing and overseeing care
coordination processes in the Novant Health Patient Oriented
Delivery System's integrated core. The supervisor helps to
facilitate the daily activities of the patient oriented delivery
system along with helping to assure ongoing quality, productivity
and efficiency in the daily operations of the Care Connection RN
Coaches. Areas of responsibility includes but not limited to
assigning care coordination processes, providing support,
management and supervision of the population health coaches and
promoting collaboration among all disciplines within the integrated
core POD as well as outside and within the community. This person
serves as a mentor/trainer and resource expert who works to ensure
safety, best practice and high quality standards of care across the
continuum.

Qualifications
- Education: Bachelor's Degree in Nursing required. 4 years /
Bachelors Degree preferred.
- Experience: One year of supervisory experience or demonstration
of leadership skills required. Care Coordination/Case Management
experience required.
- Licensure/Certification/Registration: Current RN licensure in
appropriate state required.
- Additional Skills Required: Broad clinical knowledge base of
multiple diseases and expected outcomes. Working knowledge of
effective teaching techniques, applying adult learning principles.
Clinical experience sufficient to understand the interrelationships
of complex disease processes and clinical needs of complex
patients. Ability to be self-directed as well as collaboratively
work with others. Excellent written and verbal communication.
Analytic and computer skills to include experience with Electronic
Health Record, Data and Reporting, Process and Quality Improvement
experience; PDSA Cycle activity. Demonstrates appropriate
professional nursing skills in the provision of preventive health
maintenance and/or treatment of illness. Demonstrates a working
knowledge of Novant Health and the physician office setting to act
with providing connections across the system. Demonstrate
appropriate clinical leadership skills as evidenced by previous
work related activities and performances. Demonstrate the ability
to work independently to assess and evaluate understanding of
disease processes, treatment plans and/or lifestyle changes.
Demonstrate appropriate communication, negotiating and networking
skills, Demonstrate sound knowledge and appropriate decision-making
skills for designated patient populations. Demonstrate appropriate
organization skills, Demonstrate the ability to provide program
specific outcomes. Demonstrate the ability to assess self- learning
needs and seek ways to meet own professional development by
proactively educating self, participating in orientation and staff
development activities, maintaining knowledge of clinical best
practice, issues, trends, technology and available community
resources, and uses appropriate policies and procedures. Knowledge
of applicable organizational policies and procedures; computer
systems and applications; Skill in gathering, analyzing, and
evaluating data; Skill in written and verbal communication; Ability
to apply policies and principles to solve everyday problems and
deal with a variety of situations; Ability to identify problems and
recommend solutions; Ability to establish priorities and coordinate
work activities.
