You are the ideal candidate if you are passionate about advocating for individuals in the community with a professional level of understanding of social work practices in a variety of settings. Demonstrating empathy for individuals with the need for direct services in the community you serve is a priority. Practical communication skills for interactions with clients, family members, and a range of professional service providers are an aspect of this work. You must have the ability to maintain confidentiality while problem-solving potential issues related to community services, and social barriers is beneficial to this role. Coordinate data collection to facilitate the community's needs and establish information to support individuals within communities.

For a typical assignment, the Quality of Life Specialist will receive referrals from various paths including health care providers, insurance companies, managed care plans, and other organizations working to help manage individuals' social health needs. You will coach individuals to adopt healthy behaviors, help them access community resources, and facilitate communication with health care providers and social service organizations. Through an integrated approach to case management and care coordination, you will provide social support to vulnerable populations. Job posting will remain open until filled.

POSITION REQUIREMENTS: Demonstrating empathy for individuals with the need for direct services in the community you serve is a priority. You must possess the ability to use various online case management and program monitoring databases to establish and sustain effective working relationships service, treatment plans, and plan program activities. Employee may provide general support and directive counseling to clients. Employee may serve as multidisciplinary team members and may refer clients to a variety of services to meet financial psychological, emotional, or other needs. Skills in facilitating, recruiting, and training individuals are desired with complex community services.

Bachelor's degree in human services, social work, counseling, and psychology or a related field and considerable experience with rehabilitation counseling marriage, family therapy, or another human services delivery program. Preference would be given to an individual with human services and social work experiences. An equivalent amount of education and experience sufficient to provide the required knowledge, skills, and abilities may be considered. The individual should have a working knowledge of governmental and private sector laws and regulations. Strong organizational skills, communication, verbal and written as required to interact with a variety of patrons including coworkers, general public elected officials, healthcare professionals, and other organization representatives.

SALARY RANGE: Hiring salary $44,228.29 DOQ. PTRC offers competitive salary and excellent benefits including Local Governmental Employees Retirement System (LGERS) and participates in PSLFP.
