JOB SUMMARY:

Assist patients who have been identified as victims of interpersonal violence by providing acute intervention, crisis intervention, outreach/advocacy, counseling, and intensive case management for victims of interpersonal violence and their families using a trauma focused lens. The position works both within the community and in collaboration with a multidisciplinary team to address risk factors, health disparities, and other multifaceted issues that will enable patients and their families to navigate clinical and community-based resources that will ensure optimal utilization and follow-up.

EDUCATION/EXPERIENCE:

Bachelor's degree in a healthcare-related sector (Counseling, Social Work, Sociology, Child Development, Psychology, and Nursing) required along with two years of experience working within the healthcare sector. Experience in case management, trauma services/trauma informed care, human services, violence-related injuries, criminal justice systems. Master's degree in a healthcare-related sector (Counseling, Public Health, Social Work, Psychology, and Nursing) preferred.

ESSENTIAL FUNCTIONS:

  • Meet with patients and their families by providing trauma informed emotional support, case management, and advocacy to patients who are evaluated and treated for interpersonal violence-related injuries at Atrium Health-Main Medical Center.
  • Completes screening, assessments and therapeutic support in accordance with departmental and professional standards to identify needs related to violent injuries including elements of crisis, difficulties with medications, acceptance of diagnosis, social stigma, family issues, and overall wellness.
  • Connect and link patients to resources within AHWFBH and post discharge by providing intensive case management services to assist in successful connection to community-based resources to address physical, mental, social, and emotional determinates related to overall health and wellness.
  • Maintains close communications and participates in case review and interdisciplinary team meetings within AHWFBH case managers, social workers, and other healthcare providers regarding the plan of care, patient condition and the appropriate level of intervention.
  • Maintains close communications and participates in case review with community based providers in order to sustain accurate and timely documentation of patient interactions through intensive follow-up contact with clients, family, friends and service providers.
  • Interacts and works in collaboration with patients, families, and significant others using trauma informed modalities and interventions.
  • Attends patient related appointments, meetings, and other engagements to assist patients with successful connection and navigations of community-based supports and resources.
  • Assists in department research Identify, analyze and address patient oriented trends, barriers, and challenges and develop strategies to address such by removing real or perceived obstacles.
  • Engages in supervision of clinical concerns, skills, and professional development.
  • Participates in weekly interdisciplinary meetings to communicate psycho-social issues that impact patient health condition/status.

    Ability to interact and engage with community providers, leaders, and inside and outside the AHWFB to enhance clinical and community based resources and services

    Basic computer skills

    Ability to engage with diverse populations, communities, and supporting partners.

    Ability to learn hospital systems

    Ability to use initiative, critical thinking, judgment, and discretion in daily work

    Commitment to ongoing training and development

    Bilingual in Spanish (preferred but not required)

    WORK ENVIRONMENT:

    Clean, comfortable, office environment

    Community oriented fieldwork; Travel required

    Subject to many interruptions and stressful situations
