Job Description Job Description $2,000 sign-on bonus available,
according to AHCS policy Position Summary This position is for
School-Based Case Managers who will provide case management
services to qualified children and adolescents with AHCS'
School-Based Services program. Responsibilities include linking,
coordinating, assessing, referring, and consulting during the
school day. Case managers will collaborate regularly with school
personnel, therapists, and guardians, and may provide some brief
crisis/supportive level counseling as needed. Meets with clients to
provide 1:1 interventions for support, de-escalation,
problem-solving, social skills enhancement, anxiety reduction,
anger management, et. al. Weekly supervision, and opportunities for
training are included. Required Education and Experience Bachelor's
degree from an accredited college/university in an applicable human
services field with ability to obtain QMHP. A minimum of one year
of relevant mental health, developmental disability, or substance
abuse work experience or a combination thereof that demonstrates
the candidate possesses the required knowledge, skills, and
abilities; and an equivalent combination of education, training,
and experience that provides the individual has the required
knowledge, skills, and abilities. Related Knowledge, Skills, and
Abilities Services, systems, and programs available in the
community including primary health care, support services,
eligibility criteria and intake processes, generic community
resources, mental health, developmental disabilities, and substance
use treatment programs; The use of medications in the care or
treatment of the population served; The nature of the serious
mental illness, developmental disabilities, and substance use
depending on the population served, including clinical and
developmental issues; Different types of assessments, including
functional assessments, and their uses in service planning;
Treatment modalities and intervention techniques, such as behavior
management, independent living skills training, supportive
counseling, family education, crisis intervention, discharge
planning, and service coordination; The service planning process
and significant components of a service plan; All applicable
federal and state laws, state regulations, and local ordinances.
Identifying and documenting an individual's needs for resources,
services, and other supports; Using information from assessments,
evaluations, observation, and interviews to develop individual
service plans; Identifying services and resources within the
community and established service system to meet the individual's
needs; and documenting how resources, services, and natural
supports, such as family, can be utilized to achieve an
individual's personal habilitative/rehabilitative and life goals;
Coordinating the provision of services by public and private
providers. Must possess a valid driver's license. May be called
upon to travel to any AHCS worksite; transport clients, and provide
services to clients at other locations. Regular attendance at
training and AHCS meetings may require travel to a variety of AHCS
sites; To participate in and successfully pass behavioral
intervention training; includes both verbal de-escalation skills
and physical skills techniques to be utilized with consumers as
needed; Willing and available to work modified schedules as
required; Demonstrate positive regard for individuals and their
families (e.g., allowing risk-taking, avoiding stereotypes of
developmentally disabled people, respecting individuals' and
families' privacy, believing individuals can grow). Candidates who
are offered a position must undergo a background check, including
social services review, and drug screen. Virginia has restrictions
